一般個人戶憑國民身分證、第二證件及印鑑辦理。
依你目前的狀況16歲之未成年人除了上面的東西之外，
還需要法定代理人（父母雙方）國民身分證、同意書。

父母雙方無法共同臨櫃代辦者，得依下列方式之一辦理：
1.由臨櫃之法定代理人檢附未臨櫃另一方之國民身分證及同意書單獨代理。
2.父母離婚，依協議或經法院酌定由一方行使親權者，
應提示辦妥監護登記之戶口名簿或戶籍謄本單獨代理。
3.單一監護者，應提示已辦妥監護登記之戶口名簿或戶籍謄本，由該監護人單獨代理。
